Recording form part 1 - survey square information

The National Reptile Survey form is in two parts. This is the first page, which records data about you and the square that you have surveyed. This form should be filled out only once for each of the survey squares for which you are responsible.

After you have completed this form you can then complete the second part of the survey, which records data for the animals that you have seen. This second form should be filled out once for each visit to your survey square(s). To access the second part of the form, either click here or click on the 'Record survey results' link under 'National Reptile Survey' in the left hand menu.
